I acknowledge that entry on, use of, and activity on and around property owned by The Corporation of the City of Brantford (the “City”) involves risks which may be beyond the control of the City. The risks, dangers or hazards on or about the property may result in harm, including but not limited to, broken bones, bruises, cuts, sprains, dislocations, concussions, other injuries (whether physical or mental), disability, illness and death. I voluntarily assume and accept all such risks resulting from, occurring during or being connected in any manner with my entry on the City’s property, whether caused by the acts or omissions of the City or other persons. I, and for my heirs, executors, administrators, successors, assigns and invitees, further agree to release, waive and forever discharge the City, its employees, agents, contractors, representatives, elected and appointed officials, successors, assigns (collectively the “ Indemnitees”) from any and all claims, demands, damages, costs, expenses, actions and causes of action whatsoever, whether in law or equity, in respect of death, injury, loss or damage to my person or property, however caused, arising or to arise by reason of this permission being granted for the purpose of evaluating and assessing the City’s facilities for the purposes of film activity.
I further hereby undertake to hold the Indemnitees harmless and agree to indemnify any or all of the Indemnitees from and against any and all liability incurred arising as a result of, or in any way connected with, the permission granted to me.
I further agree to pay to the City the cost of any emergency evacuation of my person or belongings that may be necessary. I affirm that I am aware of the nature of the activity, the inherent risks, and that I am properly equipped and physically able to undertake it.
By signing this statement, I acknowledge having read, understood and agreed to the above Release, Waiver of Liability, Assumption of Risk and Indemnity.
